Your Role
Duties may include:
- In-home support and personal care
- Domestic duties and light maintenance
- Community access and respite
- Participation in our weekly and weekend Day Group Programs
You may have experience in one or all of these areas — either way, we’d love to hear from you!
Requirements
- Current First Aid & CPR
- Working With Children Check (employee)
- National Police Check
- NDIS Worker Screening Check
- Driver Licence
- Must be authorised to work in Australia

Support Coordinator – NDIS (Casual, Hybrid Role)
Job Description
As a Support Coordinator, you’ll be at the heart of helping people achieve their goals. You’ll guide participants through their plans, connect them with the right services, and empower them to make choices that matter to them. Your work will help participants overcome barriers, build confidence, and engage with their community on their own terms.
Your Role
What You’ll Do
- Help participants implement their plans across funded, mainstream, community, and informal supports.
- Build relationships with participants, families, and service providers, and arrange stakeholder meetings when needed.
- Research, coordinate, and manage supports tailored to each participant’s goals.
- Identify risks, resolve barriers, and respond effectively during times of crisis.
- Support participants to self-direct their supports and engage meaningfully in their community.
- Maintain accurate case notes, reports, and documentation to support transparency and quality.
- Assist participants with plan reviews and connect them with approved funded supports.

What We’re Looking For
- Certificate IV in Mental Health or Disability (or studying / willing to obtain), or higher relevant qualification
- 12+ months experience in support coordination or case management, plus 3+ years industry or lived experience
- Current NSW Driver Licence, NDIS Worker Screening Check, Working with Children Check, and Police Check
- Strong computer skills (Microsoft Office, video conferencing, client data management systems)
- Excellent communication skills and ability to build trust with participants and stakeholders
- Understanding of NDIS, professional boundaries, billable hours, and KPIs
- A caring, professional, and non-judgmental approach to mental health and disability
